Healthy Home Agencies

There are two home health agencies based out of the Meade District Hospital.

1. We have a Medicare certified agency that offers care to Medicare, Medicaid, and Insurance patients.

• Patients admitted to this home health agency must meet the Medicare, Medicaid and Insurance requirements to qualify for these services.
• Services offered are skilled nursing, physical therapy and home health aide.
• Medicare and some insurance companies require the patient to be homebound.
• Medical necessity is needed to qualify all patients.
• The office is open 8:00 AM to 4:00 PM Monday through Friday.
• Patient care is provided any time it is needed and ordered by a physician.

2. The other home health agency is only State licensed.

• This agency serves Private Pay, Frail Elderly Home and Community Based Care, Senior Care Act, and Older American Act.
• This agency provides skilled nursing, home health aide, and attendant care.
  • Some of the services provided are medication set-up, bathing, personal care, homemaker service, cooking, shopping.
• Private Pay Care is available for anyone who needs it. Assistance will be provided to you in the home in a professional and effective manner.
• The office is open 8:00 AM to 4:00 PM Monday through Friday.
• Patient care is provided evening and weekend hours when needed.
