Good Faith Estimate 

We are committed to transparency in healthcare costs. Under the No Surprises Act, we provide Good Faith Estimates (GFEs) to individuals who are uninsured or who choose not to use their health insurance for scheduled services. This estimate outlines the expected charges for your healthcare items and services, including those from our clinic and any other providers involved in your care. 

What is a Good Faith Estimate?

A Good Faith Estimate is a projection of the costs you can expect to be billed for healthcare services. It helps you understand the potential financial responsibility before receiving care.

The estimate is based on information available at the time it is created and may not include

all potential costs, such as those arising from unexpected complications or services from

other providers. 

What's Included in a Good Faith Estimate?

A GFE will include:

  • Itemized list of services: A detailed breakdown of the services you're expected to receive. 

  • Estimated charges: The expected cost for each service, including facility fees and other associated charges. 

  • Provider information: The name, NPI (National Provider Identifier), and tax ID of the provider. 

  • Location of service: The address where the services will be provided. 

  • Potential complications: Acknowledgment that unforeseen circumstances may lead to additional costs. 

 

How to Obtain a Good Faith Estimate:

  1. Request one: You can request a GFE from our clinic by contacting us.

  2. Schedule appointments: If you schedule a service at least 3 business days in advance, we will provide you with a GFE.

  3. Review and compare: Please carefully review the estimate and keep it for your records. You can use it to compare with your final bill. 

 

Important Considerations:

  • Not a bill: The GFE is an estimate, not a final bill. Your actual bill may differ. 

  • Disputes: If your final bill is significantly higher than the GFE (by $400 or more for any provider or facility), you have the right to initiate a patient-provider dispute resolution process, according to CMS. 

  • Transparency: We are committed to providing you with the most accurate estimate possible and are here to answer any questions you may have about your healthcare costs.
