Swing Bed Services
At Meade District Hospital, our Swing Bed Program offers patients the opportunity to heal and regain independence in a hospital-based setting. Designed for individuals who no longer need acute hospital care but still require skilled nursing services, our swing bed program bridges the gap between hospital and home. Our dedicated team of nurses, therapists, and physicians provides personalized care to help patients recover. What is Swing Bed?
The Swing Bed Program is an extended care service developed by Medicare for patients who no longer need acute hospital care but are not yet ready to return home independently. The purpose of the program is to help patients regain independence so they can safely transition back to their home. Patients in the Swing Bed Program are encouraged to participate actively in their recovery by engaging in daily activities such as bathing, dressing, and other tasks that promote independence. To support this process, families are encouraged to bring in the patient’s personal clothing and care items to make their stay more comfortable and home-like.

The Swing Bed Program offers a range of skilled services designed to meet each patient’s individual needs, which may include:

Skilled Nursing Care: 24-hour nursing support for medication management, wound care, IV antibiotics, and other medical needs.

Physical Therapy: Exercises and mobility training to improve strength, balance, and endurance.

Medical and Social Services: Coordination of care, discharge planning, and patient/family education to ensure a smooth transition home.


Who Qualifies?
To qualify for the Swing Bed Program, patients must meet Medicare or insurance requirements, which generally include:

A physician’s order.

A recent inpatient hospital stay of at least three consecutive midnights in acute care.

Services must begin within 30 days of the acute stay.

Care needs must warrant skilled care, as determined by the care plan team through a needs assessment.
Needs Based Assessment
The determination to receive Skilled Care must be made by the care plan team and is based on their assessment of the patient's health care needs. The care plan team's assessment will include:

Needs or challenges resulting from the medical condition.
-
Chronic conditions that complicate the patient's acute problem.

Problems that arise from the treatment regimen.

This team meets weekly to discuss how best to meet the patient's healthcare needs. These meetings provide dedicated time to collaborate on treatments, identify needs, celebrate accomplishments, and address challenges. Care plan meetings are held on Tuesdays and Wednesdays, and we encourage the patient's family to attend.
